Application Forms

For any pension benefit to be processed, an application form must be completed and submitted to CDETB.  Each section should be completed and details of any previous service should be submitted for verification for pension purposes.
 
On the application form are a number of declarations.  If you have any other private pension or Additional Voluntary contribution plan (AVC), you should ensure that details are completed in relation to these on the application form.
 
Failure to complete the relevant sections of the application for benefits may result in a delay in processing of your pension.
